Things to know before your appointment.
We look forward to meeting you at your first visit. Here you can find some information that will help you understand what to expect and answer any questions you may have before your visit
During your first visit, you will receive a full examination including x-rays and an oral cancer screening. Dr. Daniel Le will then review your dental and medical records and the updates from the exam. Dr. Le will create a personal treatment plan for you and answer and questions or concerns you may have.
Please arrive 15 minutes before your appointment
We ask that you arrive 15 minutes prior to your appointment so you have proper time to fill out our patient forms.
Fill out patient form
Our patient forms are made to ensure we understand you and your medical history to provide you safe and personalized support. You will be asked to fill out our patient form in your preferred language (English, Spanish, Vietnamese, Chinese).
To speed up this process, you may find it useful to download, print, and fill out the form from home and bring it with you to your appointment. We have provided digital versions below for your convenience!
Note any unique health conditions you may have
Certain medical conditions and medications can influence your treatment protocols so you will be asked to provide a list of current medication and note any relevant medical conditions.
Bring a form of Personal Identification
It's important to have each patient's personal identification card on file. Make sure your personal information matches with insurance data for efficient claims processing.
